This consent waiver is stating that there is a risk with injury during exercising and KRIPTOKNIGHT-X exercise therapy & wellness center is not responsible. And that you fully understand, of legal age, & and that you exercise at your own risk.

90 DAY AGREEMENT

90 DAY AGREEMENT

I, hereby commit to a 90-day personal training program with KRIPTOKNIGHT-X ("the Trainer") beginning on.

  1. Commitment Period:

    • I agree to engage in a personal training program with the Trainer for a duration of 90 days, starting from the specified start date.
  2. Personal Training Sessions:

    • I understand that consistent attendance and active participation in these sessions are essential for achieving my fitness goals.

     

  3. Payment Terms:

    • I understand and agree to the financial commitment associated with the personal training services provided by the Trainer.
    • I agree to pay the total fee of for the 90-day training program, to be paid in installments of each, due on [Payment Due Dates].
  4. Late Payments:

    • I acknowledge that late payments may result in the suspension of personal training services.
    • I agree to make timely payments as outlined in this agreement to avoid any disruptions in my training program.
  5. Cancellation Policy:

    • I understand that cancellation of personal training sessions requires at least 2 hours' notice to the Trainer.
    • I acknowledge that failure to provide sufficient notice may result in the forfeiture of the session.
  6. Waiver of Liability:

    • I acknowledge that participation in personal training sessions carries inherent risks, including but not limited to injury or illness.
    • I voluntarily assume all risks associated with my participation in the personal training program and waive any claims against the Trainer and their affiliated entities for injuries or damages incurred during the 90-day commitment period.

By signing below, I affirm that I have read and understood the terms of this 90-Day Financial Commitment Waiver for Personal Training Services and agree to abide by them.
